NHC Family Code of Conduct, Photo and Liability Waiver

• I understand that NHC is loving, Bible-based, conservative-Christian organization. All curriculum will be taught in accordance with those guidelines.

• I will conduct myself in a manner that reflects positively on Christ, NHC, the homeschooling community, and my family. All language, conversations, and actions will be God-honoring. I will be respectful of myself, my fellow students, teachers, Epiphany staff, and all NHC patrons.

• I will respect the property of others; teachers, fellow students, NHC, and Epiphany Lutheran. If I damage any NHC or Epiphany property, I will immediately report it to my teacher. I understand that my parents will be financially responsible for any damage that I may cause, accidentally or otherwise.

• I will do my best to fulfill the requirements and expectations of the course(s) I am taking. I am responsible for my homework, papers, and belongings. I will come to class prepared with all materials and assignments that are required.

• I understand that I will dress modestly and refrain from all public displays of affection with other students while attending any NHC related activity, on or off campus.

All NHC Patrons; student, parent &/or siblings

• No NHC patrons; students, parents, or siblings are allowed outside on the playground or church lawn at any time. Everyone must use the sidewalk and wipe their feet at the entrance.

• All NHC patrons will eat in the designated area ONLY, while sitting down. No red beverages or gum is permitted. All students and parents are responsible to clean up after themselves and their young children. No students are allowed in the kitchen unless accompanied by a parent or teacher.

• All NHC patrons must stay within the NHC areas of the church. No one is permitted to linger by the entrance or past the double doors No one is permitted to run, yell or rough house. All patrons must sit quietly and use a reasonable volume.

• All NHC patrons will use God honoring language. This includes not taking the Lord’s name in vain, and refraining from gossip and slander of any teacher, student, or other NHC patron.
• All children 10 and under MUST have a parent on-site and supervising their children when they are not in class. Parents may leave the premises when students are IN class. (This is subject to change if a student is unable to conduct themselves appropriately in the parent’s absence.)

• Young children (under 10) MUST always remain with parents. The parent is responsible for any messes or behavior issues with their child(ren).


Parents and Students agree to the following discipline guidelines:
1. A meeting will take place with the parent, student, teacher, site coordinator, and an NHC board member. This will serve as a verbal warning to make the parent aware of the situation and that the teacher is no longer able to handle the situation.

2. If the issue is not resolved, a parent must sit in class with the student to monitor their behavior without interrupting the class.

3. If the issue is still not resolved, the student will be asked to not return to classes. This is WITHOUT a refund from NHC.


Photo Release
Permission to use photographs:
I grant NHC and its instructors’ permission to take photographs or video of me and my children in connection to classes and events held by NHC or homeschool related activities. I also authorize NHC to copyright, use and publish the same in print and/or electronically.
I agree that NHC may use such photographs of me and my children with or without my name for any lawful purpose, including such purposes as publicity, illustration, advertising and web content.

Cancellation & Sick Policy
For weather cancellations: If Fort Wayne Community Schools cancels school, then NHC is cancelled. If Fort Wayne Community has a delay, NHC still meets at regular time. If the weather is questionable, please use your best judgement. Safety is always our first concern. We will post cancellations on the NHC FB page and send out emails.
There is a built-in make-up day scheduled for each semester. This will only be used if all NHC classes are canceled. If there is more then one cancellation per semester, then teachers will adjust their classes accordingly. It will not be made up.
If an individual teacher cancels their classes, then the day is not made up. That teacher will adjust their classes accordingly and communicate with the students.
If your student is sick, please keep them home. Students must be fever/diarrhea/vomit free for at least 24 hours. Students should remember to cough and sneeze into a tissue or their elbow, avoid touching their faces, and reduce physical contact with others who may be sick. They should practice good hand washing practices, washing with soap and water frequently or using an alcohol-based sanitizer. It is their responsibility to contact teachers and make up any assignments missed in a timely fashion.


In consideration of NHC (Northside Homeschool Co-op) and E.L.C. (Epiphany Lutheran Church) the undersigned knowingly and voluntarily waives all claims for damage or loss to my person and property and my child(ren)’s person and property that may be caused by an act, or failure to act, of NHC and E.L.C., their officers, agents, employees, instructors, volunteers or other participants. The undersigned further assumes the risk of all conditions, which are associated with the operation of this facility and activity. In further consideration of NHC and E.L.C. the undersigned agrees to indemnify and hold NHC and E.L.C. harmless from any and all claims, lawsuits, or other legal proceedings or actions which may be brought against them as a result of any loss, damage, injury and death, sustained by myself or child(ren), in property or person, while attending this activity. I also further agree to be financially responsible for any damage caused by myself and my child(ren) to any property of NHC and their Instructors, and E.L.C., accidental or otherwise. I am the legal and responsible parent/guardian of the above child(ren). I, for myself and on behalf of my heirs, assign, personal representatives and next of kin hereby release, indemnify, and hold harmless NHC and E.L.C. I have read and understand its terms and rights I am giving up by signing below.
